This is a real equipment list with lots of technical material.  Yours will be a lot simpler!  You just need to include what is needed, when it is needed, who will be using it and who is responsible for ensuring the equipment is picked up, fully charged etc.  Don't forget to include post-production equipment.

You can create your own table for your call sheet and include actors and crew.  Basically for each scene, you need to know who is needed, what their role is, where they need to be, when they need to be there and what makeup, costume, prop and equpment requirements they might have.
